How to Register?

In order to utilize any of the services, every family has to register by doing the following:

• Fill out the application, data and agreement forms.
  
  
  
  
  
• Provide us with the child's latest complete medical checkup record and immunization history.
  
  
  
  
  
• Read our Parent's Handbook and agree to abide by the policies.
